abstract = {GATOR1 is an evolutionarily-conserved negative regulator of mTORC1-dependent signal transduction with pathogenic mutations linked to epilepsy, infantile spasms, and autism spectrum disorders. While a biochemical role of GATOR1 in amino acid-signaling is established, its cell-type specific contributions within the brain remain poorly defined. Here, we show that loss of GATOR1 function in astrocytic cells disrupts mitochondrial metabolism, with a selective dysfunction of the electron transport chain Complex II leading to elevated reactive oxygen species (ROS) and redox imbalance. These changes are accompanied by compensatory increases in antioxidant regulatory systems including superoxide dismutase, but remain insufficient to ameliorate the increased ROS. GATOR1-deficient astrocytes show metabolic rewiring marked by enhanced expression of glutamate uptake and glutamine synthesis pathways that contribute to the glutamate-glutamine cycle governing neuronal glutamine availability and synaptic homeostasis. In vivo, GATOR1 deficiency results in progressive astrocytic reactivity, seizures, and a reduced lifespan. These findings demonstrate that GATOR1 function is critical to coordinate astrocytic mitochondrial activity and neurotransmitter cycling pathways, establishing a novel link between intracellular amino acid-signaling in astrocytes and excitatory neural network homeostasis.},

abstract = {According to the entrainment region hypothesis, the range of tempi with which individuals can synchronize broadens during childhood. This developmental change is accompanied by a slowing of rhythmic preferences, as covered by the preferred period hypothesis. The later hypothesis posits that both motor and perceptual rhythmic preferences slow down throughout childhood, reflecting an increase in the common period of endogenous oscillations. This study aimed to further investigate the development of an entrainment region through rhythmic production (counting) and perception (tempo discrimination), while assessing the related contributions of a preferred period (spontaneous motor tempo and perceptual preferred tempo) between childhood and young adulthood. The study (N = 70) included three groups of children (5-6, 8-9, and 11-12 years) and one group of young adults (21-30 years). The results show a change in rhythmic performances around 11-12-year-olds, depending on the task employed. Moreover, results indicate a significant effect of rhythmic context in tempo discrimination, suggesting that young children are sometimes better at discriminating tempo slower than their motor and perceptual rhythmic preferences. For both rhythmic production and perception, spontaneous motor tempo emerges as the strongest contributor to the models, reinforcing its validity as an index of the biological constraints associated with endogenous oscillation. In contrast, the opposite direction of age across models suggests distinct developmental trajectories for rhythmic production and perception, highlighting the influence of additional constraints beyond endogenous oscillation (e.g., cognitive).},

abstract = {Midbrain dopamine (mDA) neurons play a wide range of brain functions, but the molecular mechanisms driving the formation of mDA circuits remain largely unknown. Here, we show that autophagy, the main cellular recycling pathway, is present in the growth cones of developing mDA neurons, and its level changes dynamically in response to guidance cues. To characterize the role of autophagy in mDA axon growth and guidance, we knocked out essential autophagy genes (Atg12, Atg5) specifically in mDA neurons in mice of either sex. Autophagy-deficient mDA axons exhibit axonal swellings and reduced branching both in vitro and in vivo. Strikingly, deletion of autophagy-related genes completely blunted the response of mDA neurons to both chemorepulsive and chemoattractive guidance cues. Our data demonstrate that autophagy plays a central role in regulating mDA neuron development by orchestrating axonal growth and guidance. },

abstract = {Aging is the most impactful risk factor for cognitive decline. The prefrontal cortex (PFC), a brain region essential for higher-order cognition, undergoes age-related synaptic alterations which are postulated to yield an excitation/inhibition imbalance within the PFC. Here, we assessed cognitive performance in young and aged mice using a battery of behavioral tests linked to PFC function. Behavioral variability within the aged cohort suggested structured heterogeneity in cognitive performance. Using a data-driven analytical framework integrating behavioral dimensionality reduction, consensus clustering, and spectral embedding we achieved robust and stable behavioral subgroup separation within the aged population. This approach revealed a subset of "cognitively susceptible" aged mice with pronounced memory and novelty-directed exploration deficits, distinct from both "resilient" aged mice and young controls. Unlike resilient aged mice, susceptible mice showed intact social preference. Susceptible aged mice exhibited elevated levels of the inhibitory synaptic proteins Gephyrin and VGAT in the PFC, a pattern absent in resilient aged mice. Notably, the increase in Gephyrin was accounted for by a higher density of inhibitory synapses, indicating a structural shift toward inhibition. Consistent with this mechanism, optogenetically enhancing inhibition in the PFC of young mice was sufficient to recapitulate the memory and novelty-exploration deficits observed in susceptible aged mice. Altogether, these findings implicate excessive prefrontal inhibition as a mechanistic substrate contributing to cognitive susceptibility in unsuccessful aging.},

abstract = {INTRODUCTION: The Positron Emission Tomography (PET) tracer C-PBR28 binds to the Translocator Protein (TSPO), a marker of activated microglia. Advanced Magnetic Resonance (MR) techniques are sensitive to different tissue properties such as myelin (myelin water fraction, MWF; magnetisation transfer ratio, MTR; radial diffusivity, RD; choline, tCho), axons (axial diffusivity, AD; n-acetylaspartate, tNAA) and gliosis (myo-inositol, mI).nnOBJECTIVE: To compare C-PBR28 binding across MS subtypes and investigate the correlation between C-PBR28 binding and advanced MR measures in normal-appearing white matter (NAWM) and lesions.nnMETHODS: Twelve people living with MS (7 relapsing-remitting MS (RRMS) and 5 progressive MS (PMS)) and 6 healthy controls (HC) were scanned with PET and MRI. A 70-minute C-PBR28 PET session was used to determine the non-displaceable binding potential (BP). Advanced 3T MR included myelin water imaging, diffusion tensor imaging and spectroscopy. Mean measurements were extracted from lesion and NAWM masks.nnRESULTS: Within NAWM, C-PBR28 binding was different between groups (BP HC=0.70, RRMS=0.72, PMS=0.79; p = 0.03) and lesions exhibited lower BP (NAWM=0.75, lesion=0.63; p = 0.0002). Lesions also had lower MWF (NAWM=0.13, lesion=0.08) and MTR (NAWM=41.5, lesion=33.5), and higher AD (NAWM=1.05, lesion=1.41) and RD (NAWM=0.50, lesion=0.83) than NAWM (all p < 0.0001). In NAWM, correlations were found between PBR-BP and MWF (R=-0.71, p = 0.001) and MTR (R=-0.68, p = 0.003).nnCONCLUSIONS: Increased inflammation and gliosis indicated by elevated C-PBR28 binding in NAWM are associated with demyelination (decreased MWF and MTR), especially in PMS. The ability to measure and monitor diffuse inflammation could help evaluate therapies designed to target whole brain immune activity and more specifically microglia.},

abstract = {Opioid analgesia and adverse outcomes vary across individuals. We show that runt-related transcription factor 1 (Runx1) modulates the microglial transcriptome and is a genetic determinant of opioid antinociceptive responses and withdrawal. In mice, Runx1 deletion in microglia produces distinct ultrastructural and transcriptomic signatures, reducing morphine potency despite no prior opioid exposure. These mice also require greater post-operative morphine and display exacerbated morphine-induced hyperalgesia and withdrawal. Single-cell RNA sequencing (scRNA-seq) and chromatin immunoprecipitation sequencing (ChIP-seq) analyses reveal a unique microglial state, with Runx1 regulating inflammatory signaling and key microglial functions. In humans, association analyses link RUNX1 variants to inter-individual differences in perioperative opioid requirement and withdrawal severity. Identifying RUNX1 susceptibility genotypes may be important for understanding individual variability in opioid responses, with potential relevance for future personalized approaches.},

abstract = {BACKGROUND AND HYPOTHESIS: First-episode psychosis (FEP) often coincides with comorbidities that hinder recovery. Gaming disorder (GD) has emerged as a potential concern; however, data on its prevalence and clinical impact in FEP populations remain limited. We hypothesized that GD would be more prevalent among individuals with FEP than in the general population and associated with more severe psychotic symptoms and poorer functional outcomes.nnSTUDY DESIGN: In this prospective cohort study, 284 consecutive patients aged 18-35 years admitted to an Early Intervention Service in Quebec, Canada, were assessed for GD using International Classification of Diseases-11 criteria, with up to 24 months follow-up. Psychotic symptoms and functioning were measured at baseline and at 6-month intervals using the Positive and Negative Syndrome Scale-6 items (PANSS-6) and the Social and Occupational Functioning Assessment Scale (SOFAS).nnSTUDY RESULTS: Gaming disorder was present in 7.04% (95% CI: [4.35%, 10.67%]) of the sample, exceeding the 3.3% general population estimates. Among the 105 patients who reported gaming at least monthly, those with GD (n = 20) more often played Massively Multiplayer Online Role-Playing Games, while non-GD gamers (n = 85) preferred action-adventure games. GD was associated with significantly higher PANSS-6 scores (P = .04), driven by elevated negative symptoms (P = .04). SOFAS scores were consistently lower among individuals with GD, with effect sizes increasing over time.nnCONCLUSIONS: Gaming disorder is a prevalent and clinically meaningful comorbidity in FEP, linked to greater negative symptom burden and functional impairment. These findings support the need for systematic GD screening in early psychosis services and highlight the importance of developing tailored interventions.},

abstract = {BACKGROUND: The intrinsic cardiac nervous system is a complex system that plays a critical role in the regulation of cardiac physiologic parameters and has been shown to contribute to cardiac arrhythmias. To date, several types of neurons with distinct neurochemical and electrophysiologic phenotypes have been identified. However, no study has correlated the neurochemical phenotype to a specific electrophysiologic behavior. Calbindin-D28k, a calcium-binding protein, is expressed in numerous cardiac neurons.nnOBJECTIVE: Given that changes in neuronal excitability have been associated with arrhythmia susceptibility and that calbindin expression has been associated with modulations of neuronal excitability, our objective was to assess whether the cardiac calbindin neuronal population has specific properties that could be involved in cardiac regulation and arrhythmias.nnMETHODS: By using a Cre-Lox mouse model to specifically target calbindin neurons with a fluorescent reporter, we characterized the neurochemical and electrophysiologic phenotype of this cardiac neuronal population.nnRESULTS: Calbindin neurons exhibit a specific neurochemical expression profile and a larger soma with shorter neurite length compared with other neurons. This was combined with a distinct electrophysiologic signature characterized by a lower excitability with a predominantly phasic profile associated to a lower N-type calcium current density.nnCONCLUSION: Calbindin cardiac neurons display distinct neurochemical, morphologic, and electrophysiologic properties, resembling the cardiac neuronal remodeling observed in pathologic processes such as heart failure. Therefore, we believe that this specific neuronal population deserves investigation in the context of cardiac diseases.},

abstract = {Schizophrenia spectrum disorders (SSD) are marked by working memory impairments associated with abnormal functional brain connectivity. Although transcranial magnetic stimulation (TMS) shows promise in modulating dysconnectivity patterns and improving cognitive symptoms, current protocols often lack target personalization, overlooking significant variability in functional network topography between individuals. Twenty-two individuals with SSD and cognitive deficits underwent 20Hz repetitive TMS to the left lateral prefrontal cortex. Personalized TMS targeted regions with the strongest central executive-default mode network (CEN-DMN) antagonism, while standardized TMS focused on the EEG F3 site. Resting-state fMRI scans were conducted pre- and post-TMS sessions to evaluate changes in CEN-DMN connectivity, and working memory performance was assessed after the post-TMS fMRI scan. Both TMS protocols failed to significantly alter CEN-DMN connectivity or improve cognitive function, which may be due to the low reliability of the biomarker used for personalized targeting. However, stronger DMN intra-network connectivity at the stimulation site was positively correlated with a reduction in CEN-DMN connectivity and improved working memory performance. These findings highlight the need for more extensive fMRI data for better target determination, and suggest that targeting left prefrontal areas with higher DMN connectivity could more effectively modulate functional connectivity and improve working memory performance through TMS.},

Time-varying Cox regression models were used to estimate hazard ratios (HRs) for candidate predictors.nnRESULTS: Among 520 individuals without prior PBG (mean age = 24.6±4.0 years; 28.8% women), 18 developed PBG during a mean follow-up of 478 days, yielding an incidence rate of 2.6 cases/ 100 person-years. In site-adjusted analyses, white ethnicity (HR = 9.7; 95%CI = 1.3-74.8), incomplete high school education (HR = 2.8; 95%CI = 1.1-7.2), stimulant use disorder (HR=2.8; 95%CI = 1.0-7.3), use of D2/D3-5-HT1A partial agonists (HR = 4.6; 95%CI = 1.5-14.1), and prior non-problematic gambling (HR = 3.1; 95%CI = 1.1-8.4) predicted increased risk. Thirteen cases occurred during aripiprazole treatment, which remained associated with increased PBG risk after multivariable adjustment (adjusted HR = 4.7; 95%CI = 1.6-13.9).nnCONCLUSIONS: Despite the limited number of incident cases, these results suggest that PBG is relatively common PBG and is associated with potential risk factors, including white ethnicity, incomplete high school education, stimulant use disorder, prior non-problematic gambling, and treatment with D2/D3-5-HT1A partial agonists, particularly aripiprazole. These findings underscore the importance of routine screening for PBG and risk-informed antipsychotic prescribing in FEP.},

abstract = {Several studies have explored how the duration of stimuli delimiting time intervals influences interval judgments and perceived duration. Most focus on nonneighboring intervals and rely on two-alternative forced choice methods, while the method of adjustment and multimodal conditions are rarely used. This study investigated the ability to discriminate two neighboring intervals using an equisection and adjustment method, with variations in the duration of the stimuli marking the intervals, under multimodal conditions. In two experiments, participants adjusted the second of three brief successive signals (10 or 100 ms) marking two empty intervals until they were perceived as equal. In Experiment 1, only the second signal was lengthened to 100 ms. In Experiment 2, the first, third, or both the first and third signals were lengthened. Results from Experiment 1 show that interval judgments are more variable only when the second marker is lengthened within the same modality. Moreover, in intermodal conditions, the interval following the lengthened marker is perceived as longer than the preceding one. Experiment 2 reveals no overall difference in variability across marker-lengthening conditions but consistently shows that the interval following a lengthened marker is perceived as longer, regardless of modality. These findings clarify how stimulus duration and modality influence equisection judgments of short time intervals, shedding light on when and how timing mechanisms operate.},

abstract = {Sex and gender are important variables in research, but they are inconsistently explored. The international PAINDIFF Network makes 13 recommendations for studying sex and gender as variables in pain research, which are applicable across the spectrum of biopsychosocial research. Five universal recommendations apply to the majority of research studies: (1) include males and females as standard practice, (2) account for sex in randomization or counterbalancing and testing order, (3) power for sex differences when sex is a primary experimental variable, (4) include detailed reporting of experimental design, and (5) conduct sex-disaggregated analysis and reporting. Three additional recommendations specifically for preclinical studies and five additional recommendations for human and clinical studies are included. Recommendations for stakeholders, such as editors, reviewers, funding bodies and policymakers, have also been developed. Wide adoption and implementation of these recommendations will reduce variability, improve reproducibility and enhance the translatability of research findings within and beyond the field of pain.},

abstract = {We present the University of California San Diego post-treatment glioblastoma (UCSD-PTGBM) annotated multimodal MRI dataset. The UCSD-PTGBM dataset includes 243 timepoints on 178 subjects with histopathologically-proven glioblastoma who were imaged with an advanced brain tumor protocol on 3 Tesla MRI scanners. Sequences include standard 3D imaging, as well as multishell diffusion (Restricted Spectrum Imaging, RSI) and perfusion imaging techniques (Arterial Spin Labelling, ASL and Dynamic Susceptibility Contrast, DSC), and neuroradiologist approved voxelwise tumor segmentations for both traditional segmentation masks and cellular tumor segmentations. The dataset also includes isocitrate dehydrogenase (IDH) mutation status and O6-methylguianine-DNA methyl-transferase (MGMT) promotor methylation status, as well as overall survival and progression free survival information for a subset of cases. We hope that researchers around the world will use these data to continue to improve analysis of post-operative MRI on glioblastoma patients, translate their findings into clinical practice and improve the management and outcome for these patients.},

Effective radiotherapy planning for meningiomas necessitates accurate and consistent segmentation of target volumes on MRI, a process that is complex, labor-intensive, and dependent on expert expertise. The 2024 Brain Tumor Segmentation Challenge Meningioma Radiotherapy (BraTS-MEN-RT) Dataset addresses this problem by providing the largest multi-institutional collection of systematically annotated radiotherapy planning MRIs for meningiomas. Publicly accessible, this dataset comprises 570 radiotherapy planning 3D T1-weighted post-contrast MRIs at native resolutions, with 500 cases featuring expert-annotated gross tumor volumes (GTV). Annotations follow standardized radiotherapy planning protocols and include both intact and postoperative meningioma cases, ensuring wide clinical relevance. Contributions from seven diverse medical centers across the United States and the United Kingdom enhance the dataset's generalizability. The dataset aims to accelerate the development of automated segmentation methods for radiotherapy planning, improving workflow efficiency, reducing interobserver variability, and ultimately enhancing patient outcomes.},

abstract = {INTRODUCTION: Immersive virtual reality (VR) has attracted growing interest in long-term care (LTC) as a potential tool to enhance well-being and alleviate pain. However, its effective implementation by LTC staff remains understudied. In a participatory action, mixed-methods study co-developed with knowledge users (LTC frontline staff and managers, and a VR developer), we applied the Decomposed Theory of Planned Behaviour (DTPB) to examine VR adoption in LTC and clarify barriers and facilitators to sustainable implementation.nnMETHODS: Knowledge users were consulted to design and develop a single-phase, cross-sectional, convergent mixed-methods study. LTC staff ( = 16) were then recruited to complete an online self-report questionnaire specific to staff adoption of VR through the assessment of attitudes, social norms, perceived behavioural control and facilitating conditions. Quantitative data were analyzed descriptively. Qualitative data underwent content analysis via DTPB-guided deductive and inductive codes.nnRESULTS: LTC staff indicated overall favourable attitudes towards VR use, yet only 25% ( = 4) of participants rated VR as easy to use and 50% ( = 8) deemed it suitable for LTC settings. Staff confidence with VR use was moderate, with resident selection and troubleshooting highlighted as particular challenges. Barriers such as lacking time to learn and use VR systems, as well as potential resident discomfort, were highlighted. Facilitators included adequate activity space, organizational support, and person-centred delivery, which staff linked to residents' relaxation, positive affect, and progressive engagement. Content analysis identified four key themes explaining these patterns: (1) Barriers outweighing promise, (2) Personalization of the VR experience, (3) Enabling conditions-Environment and organization, and (4) Staff ambivalence to VR.nnDISCUSSION: VR adoption appears to be linked to alignment between compatibility, ease-of-use, facilitating conditions, and social support. Person-centred delivery and organizational support may enable consistent use and enhance residents' relaxation, positive affect and progressive engagement, relevant to pain care. Findings offer a practical roadmap for integrating VR as a low-burden intervention, while highlighting areas needing further attention to promote sustainable implementation.},

abstract = {Long COVID is characterized by persistent symptoms and physiological impairments beyond acute infection. Oxygen consumption (VO₂) and ventilatory efficiency, key indicators of cardiorespiratory fitness, are commonly diminished in this population, contributing to exercise intolerance. This study compared cardiorespiratory patterns and exercise capacity among individuals with Long COVID (LCG), those with resolved symptoms (Short COVID; SCG), and healthy controls (CG). The secondary objective was to assess longitudinal changes over six months. Cross-sectional comparisons at baseline addressed the primary objective, while the longitudinal component explored on changes over time. Participants included 94 in the LCG, 100 in the SCG, and 70 in the CG. All performed a 6-minute walk test (6MWT) during which peak oxygen uptake (VO₂), minute ventilation (VE), and VE/VCO₂ (ratio of VE to carbon dioxide output, reflecting ventilatory efficiency) were continuously measured using the Metamax 3B system. Baseline differences were assessed by one-way ANOVA, and longitudinal changes by generalized estimating equations. At baseline, VO₂ was significantly reduced in the LCG (70.1 % predicted) versus SCG (80.6 %) and CG (84.6 %) (p = 0.001). VE/VCO₂ was elevated in the LCG (30.2 ± 4.2) compared to SCG (27.9 ± 2.8) and CG (28.7 ± 3.3) (p < .001). The 6MWT distance was also lower in the LCG (484 ± 127 m) than SCG (607 ± 69.1 m) and CG (571 ± 89.6 m) (p < .001). No statistically significant Group × Time interaction emerged between the groups. Individuals with Long COVID exhibited persistent ventilatory inefficiency and reduced exercise capacity, with impairments persisting over six months, underscoring the need for targeted rehabilitation. SHORT ABSTRACT: Individuals with Long COVID exhibit reduced exercise capacity and persistent ventilatory inefficiency compared to Short COVID and control groups. At baseline and over six months, VO₂, VE/VCO₂, and 6MWT distances were significantly impaired, highlighting the need for targeted rehabilitation strategies to address ongoing physiological limitations in this population.},

abstract = {Study designProspective multicentric study.ObjectiveThoracolumbar fractures without neurologic deficit are challenging situations in terms of treatment decision making. We aimed to analyze the occurrence of adverse events (AEs) after surgical and nonsurgical treatment and its impact on functional outcomes.Methods198 patients from a prospective multicentric database were included. The occurrence of adverse events and treatment failure within 2 years of follow up were recorded. ODI was compared between patients with and without adverse events at six months, 1 year and 2 years follow up. Multivariable regression analysis was conducted to assess the association between post-treatment adverse events and ODI at 1-year follow-up.Results46 adverse events were recorded (23.2%). Higher categories of the Charlson Comorbidity Index (CCI) ( = 0.006), unemployment or retirement ( = 0.027), and current smoking ( = 0.008) were significantly associated with the occurrence of adverse events whereas no significant differences were observed in terms of treatment decision (conservative vs surgical). ODI values were significantly higher in patients with adverse events at the 6-month and 1-year follow-up visits without significant differences at 2 years follow up. Treatment failure was observed in only 5 patients.ConclusionWe found association between the occurrence of AE and higher ODI at 6-months and one-year follow up. Additionally, a higher CCI and smoking status were associated with higher likelihood to develop adverse events in our cohort.},

This study aimed to further document the psychometric properties of the COST and the SKT and the acceptability of these tests in people with SSD and community controls (CO).nnMETHODS: Forty-four (44) participants with SSD and 49 CO were administered the COST and SKT twice, about 4 weeks apart, and were asked to rate the acceptability of the tests on a 0 (Very unpleasant) to 10 (Very pleasant) point scale at both timepoints.nnRESULTS: In both groups, the results revealed an excellent inter-rater reliability and a good test-retest reliability for both tests, though the control non-social reasoning measure included in the COST showed poorer test-retest reliability in the SSD group. Some practice effects were observed but the ToM score from the COST and the SKT total score showed no evidence of ceiling effects at either timepoints. The average acceptability scores ranged between 7.8/10 and 8.3/10 for the COST and between 6.8/10 and 7.9/10 for the SKT.nnCONCLUSION: The SKT and the COST present with good psychometric properties, representing good options for future studies or for use in clinical practice.},

abstract = {Kir (inwardly rectifying potassium) channels that play key roles in maintaining potassium homeostasis, neuronal excitability, and osmoregulation have been cloned and characterized in a variety of insects. In , three Kir channels (dKir1 dKir2, and dKir3) have been cloned and characterized, and share significant homology with mammalian Kir channels. The dKir channels are essential for various developmental processes, such as wing patterning, by modulating bone morphogenetic protein signaling pathways. Electrophysiological studies have confirmed that Kir channels function in a way analogous to their mammalian counterparts, indicating that their roles in cellular and developmental signaling have been evolutionarily conserved. Several Kir channels have also been identified and characterized in mosquitoes ( and ). Interestingly, insect Kir channel orthologs cluster into three gene "clades" or subfamilies (Kir1, Kir2, Kir3) that are distinct from mammal Kir channels based on sequence comparisons. Insect Kir channel paralogs range from two to eight Kir channel genes per species genome representing separate gene duplication events. These differences may be attributed to distinct physiological adaptations associated with their respective taxonomic groups. The honeybee genome contains two Kir channel genes, AmKir1 and AmKir2, producing six Kir channel isoforms via alternative splicing, which have been cloned and expressed in heterologous systems to study their electrophysiological properties. This review provides a comprehensive overview of current knowledge about Kir channel structures, activities, and gating as well as of their roles in insects, including evolutionary genomic aspects, molecular biology, physiological roles, and pharmacological targeting.},

abstract = {The factors influencing the establishment of the gut bacterial community in early life are fairly well studied. However, the factors shaping the infant gut virome remain elusive. Interestingly, early life gut virome imbalances have recently been linked with increased risk of developing diseases like type 1 diabetes and asthma. We utilized the deeply phenotyped COPSAC2010 cohort to investigate how environmental factors influence the gut virome at one year age. We demonstrate that the presence of older siblings as well as residential location (urban or rural) had the strongest impact on gut virome composition at 1 year of age. A total of 16,118 species-level clustered viral representative contigs (here termed viral Operational Taxonomic Units - vOTUs) were identified and of these 2105 vOTUs varied in abundance with environmental exposures. Of these vOTUs 94.1% were phages mainly predicted to infect , , and . Strong co-abundance of phages and their bacterial hosts was confirmed underlining the predicted phage-host connections. Furthermore, we found some gut viruses affected by environmental factors encode enzymes involved in the utilization and degradation of major dietary components, potentially affecting infant health by influencing the bacterial host metabolic capacity. These findings provide a valuable insights for understanding the early life factors that predispose to autoimmune and metabolic disorders.},

abstract = {INTRODUCTION: MRI is increasingly recognised as a valuable tool for assessing prognosis and predicting outcomes following traumatic spinal cord injury (SCI). Several potential MRI biomarkers have been identified, but efforts are still needed to improve the accuracy and feasibility of these biomarkers in clinical practice. This study aims to build a national Canadian SCI imaging repository for storing and analysing imaging data for SCI, with the goal of improving SCI MRI biomarkers to predict outcomes and inform clinical management.nnMETHOD AND ANALYSIS: As a substudy of the Rick Hansen SCI Registry (RHSCIR), this retrospective multisite study includes individuals who sustained a traumatic cervical SCI between 2015 and 2021, were previously enrolled in RHSCIR, and had MRI scans acquired within 72 hours of injury and before any surgical intervention. Individuals with a penetrating trauma and/or with any prior spine surgery are excluded. The study principal investigator and research associates, experienced with data curation and with the standardised format and specifications of the Brain Imaging Data Structure standard, guide the site's curator on the steps to perform image deidentification and curation to create standardised datasets across all sites. These datasets are transferred to a Digital Research Alliance of Canada ('the Alliance') server designated for this project and concatenated to form the national Canadian SCI imaging repository (Neurogitea). We are using a semiautomated processing pipeline to quantify lesion morphology, together with additional imaging measures that are manually extracted from the images (for instance, the relative maximal spinal cord compression and the maximum canal compromise). Through linkage to RHSCIR clinical and epidemiological data already available on eligible participants, regression analysis is planned to predict neurological outcomes at discharge, including the American Spinal Injury Association Impairment Scale grade, upper and lower extremity motor and sensory scores.nnETHICS AND DISSEMINATION: This protocol has been submitted by the participating sites to obtain ethics and institutional approvals prior to the study initiation at each site. All 12 sites across Canada have now obtained ethics and institutional approvals. Study results will be disseminated at local, national and international conferences and by journal publications.},

This perspective article focuses on the intersection of GD and FEP, highlighting key diagnostic and treatment challenges, including symptom overlap that complicates differential diagnosis, the absence of validated screening tools, and difficulties in sustained patient engagement. Drawing insights from substance use disorder management in FEP, we propose a preliminary clinical framework for integrating GD assessment and intervention into early intervention in psychosis programs. This approach prioritizes comprehensive evaluation, patient-centered care, and a harm-reduction model that supports digital well-being. Addressing GD inFEP populations is crucial for optimizing functional recovery and promoting a holistic, recovery-oriented approach to psychiatric care. abstract = {OBJECTIVE: The objective of this study was to determine minimal clinically important difference (MCID) values for patient-reported outcomes (PROs) including the 12-Item Short-Form Health Survey (SF-12) Physical Component Summary (PCS), SF-12 Mental Component Summary (MCS), and Neck Disability Index (NDI) in patients with degenerative cervical myelopathy (DCM) undergoing surgery, and to assess whether MCID values vary by baseline disease severity.nnMETHODS: The authors retrospectively analyzed prospectively collected data from the Canadian Spine Outcomes and Research Network for DCM patients treated surgically between 2015 and 2023. Inclusion required a baseline modified Japanese Orthopaedic Association (mJOA) score and 3- or 12-month follow-up PROs with domain-specific anchor responses. Patients were stratified by baseline mJOA score into mild (score ≥ 15), moderate (score 12-14), and severe (score < 12) groups. MCID values for the SF-12 PCS, SF-12 MCS, and NDI were calculated using anchor-based receiver operating characteristic curve analysis, with responder status defined by anchor questions. Discriminative performance was assessed via area under the curve, and 95% confidence intervals were estimated by bootstrapping.nnRESULTS: Among 290 patients meeting inclusion criteria, 77 (26.6%) were classified as having mild myelopathy, 120 (41.4%) moderate, and 93 (32.1%) severe. In the overall cohort, the MCID values were estimated as 8.9 (95% CI 7.5-10.9) for SF-12 PCS, 4.3 (95% CI 2.3-5.6) for SF-12 MCS, and 13.5 (95% CI 11.5-15.5) for NDI. Stratified SF-12 PCS MCID values increased from an estimated 4.8 (95% CI 1.1-7.7) in mild cases to 8.4 (95% CI 6.1-11.3) in moderate and 14.8 (95% CI 10.4-17.7) in severe cases. The NDI MCID values similarly rose from 10.5 (95% CI 6.5-12.5) to 15.0 (95% CI 10.5-19.0) to 17.5 (95% CI 14.5-21.0) across the mild, moderate, and severe groups, respectively. In contrast, the SF-12 MCS MCID values were 4.5 (95% CI 1.4-7.4) for mild, 3.8 (95% CI 0.4-5.8) for moderate, and 4.4 (95% CI 1.9-8.3) for severe patients, which did not differ significantly across severities.nnCONCLUSIONS: MCID values for PROs in DCM patients undergoing surgery increase with baseline severity. These findings indicate the importance of stratifying patients by disease severity to enhance the clinical relevance of MCID values, facilitate personalized treatment goals, and improve outcome assessments.},

abstract = {The known diversity of CRISPR-Cas systems continues to expand. To encompass new discoveries, here we present an updated evolutionary classification of CRISPR-Cas systems. The updated CRISPR-Cas classification includes 2 classes, 7 types and 46 subtypes, compared with the 6 types and 33 subtypes in our previous survey 5 years ago. In addition, a classification of the cyclic oligoadenylate-dependent signalling pathway in type III systems is presented. We also discuss recently characterized alternative CRISPR-Cas functionalities, notably, type IV variants that cleave the target DNA and type V variants that inhibit the target replication without cleavage. Analysis of the abundance of CRISPR-Cas variants in genomes and metagenomes shows that the previously defined systems are relatively common, whereas the more recently characterized variants are comparatively rare. These low abundance variants comprise the long tail of the CRISPR-Cas distribution in prokaryotes and their viruses, and remain to be characterized experimentally.},

abstract = {BACKGROUND: This multi-centric study aimed to explore differences in brain activity patterns in patients with disorders of consciousness (DoC), including unresponsive wakefulness syndrome (UWS) and minimally conscious state (MCS).nnMETHODS: Using high-density electroencephalographic (EEG) recordings from 368 DoC patients, 39 who emerged from MCS (eMCS), and 73 healthy controls, we examined instantaneous functional connectivity-based meta-states acting as attractors in a dynamical system, extracted by means of community detection algorithms and recurrence analysis. We analyzed data from two patient cohorts and included resting-state and auditory processing tasks in four frequency bands (delta, theta, alpha, beta) and from three perspectives, namely: (i) discrete activation of dominant states, (ii) a dynamical system composed of attractor states and (iii) the correlation and anticorrelation patterns of the active states.nnRESULTS: Findings revealed that while the overall structure of brain connectivity remained stable after injury, patients with DoC and those who emerged showed notable differences in the speed and consistency of how their brain states activated. Specifically, in higher frequencies, UWS patients exhibited faster, and less stable dynamics, shorter dwell times and decreased meta-state anticorrelation compared to those in MCS and eMCS. Moreover, a four-way combined learning classification analysis showed that the measures were able to distinguish the UWS and MCS subgroups.nnSIGNIFICANCE: These brain state dynamics could serve as valuable markers for assessing states of consciousness. Our results highlight the potential of using high-temporal resolution dynamic brain activity patterns to improve the understanding of altered consciousness and their application to clinical settings.},

abstract = {OBJECTIVES: The present study evaluated associations between social environment profiles and cognitive outcomes among generally healthy adults using cross-sectional data from the Canadian Longitudinal Study on Aging (CLSA) Comprehensive Cohort ( = 19,793).nnMETHOD: Latent class analysis classified participants into social environment profiles based on social network size, social support, social cohesion, and social isolation. Three profiles emerged, which were labeled as representing weaker, intermediate, and stronger social environments (16.6%, 40.4%, and 42.9% of the sample, respectively). Scores on eight cognitive tests were combined into three domains: executive function, episodic memory, and prospective memory. Analyses of covariance assessed associations between the social environment profiles and executive function and episodic memory, while prospective memory was assessed with logistic regression.nnRESULTS: Significant associations between the social environment profiles and the three cognitive domains were observed among all statistical models (all ≤ 0.001). Executive function and episodic memory scores significantly differed between all three profiles, while prospective memory differed between the weaker and stronger profile. The effect sizes of associations were weak, potentially reflecting the generally cognitively healthy sample.nnCONCLUSION: The social environment is linked with cognitive functioning, but further research is needed to assess the clinical relevance and utility as a target for health promotion.},

abstract = {BACKGROUND: Rotator cuff-related shoulder pain (RCRSP) is a common musculoskeletal disorder, with prevalence increasing with age. In Aotearoa New Zealand, many people with RCRSP are compelled to seek physiotherapy privately as hospital outpatient clinics have long waiting lists, or go without care. We test the efficacy of Stepped Care, a new approach to RCRSP primary care, to use resources as efficiently as possible. This study aims to (i) define the effectiveness of a Stepped Care Model for persistent RCRSP compared to usual, pragmatic physiotherapy; (ii) explore patients' and the physiotherapists' perspectives and experiences of the interventions and research processes; and (iii) compare the cost-effectiveness of the Stepped Care Model with pragmatic physiotherapy.nnMETHODS: This pragmatic, two-armed parallel-group randomised clinical trial will enrol 200 adults with RCRSP, who are randomised to receive either Usual Physiotherapy or Stepped Care. In the Stepped Care group, all patients receive two 1-h sessions focussing on patient education. After 6 weeks, those with persistent symptoms receive pragmatic physiotherapy, while those whose symptoms have resolved will receive no further intervention. The Usual Physiotherapy group receives up to 10 sessions of pragmatic care over a 3-month period. The primary outcome is upper limb function and severity of symptoms using the QuickDASH at 3 months. Secondary outcomes includes pain severity, pain-related fear, pain self-efficacy and health-related quality of life, assessed at baseline, 6 weeks, 3, 6, and 12 months. Outcome measures in continuous scale, including QuickDASH, will be analysed to estimate the difference in the measure between two physiotherapy treatments, while the measures in categorical/binary scales will be analysed to estimate the differences of proportions with favourable outcomes. Confidence intervals will be used to make noninferiority interpretations. A cost-effectiveness analysis will be conducted at the individual level and reported from a societal perspective.nnDISCUSSION: If the findings from this study support a stepped care approach to treatment for RCRSP, it could lead to more treatment being available at lower cost to both individuals and the health system.nnTRIAL REGISTRATION: Universal Trial Number: U1111-1298-4087. abstract = {PURPOSE: To determine the internal consistency, test-retest reliability, construct validity, and responsiveness of the Brief Resilience Scale (BRS) in a sample of adults with persistent rotator cuff-related shoulder pain (RCRSP).nnMATERIAL AND METHODS: A total of 115 adults with persistent RCRSP participated in the study. All participants completed the BRS at baseline and again at 12 weeks to evaluate responsiveness. Of these, 92 completed the BRS 1-2 weeks after baseline to evaluate test-retest reliability. Additionally, construct validity was investigated through a-priori hypothesis testing, comparing the BRS with patient-reported outcome measures that assess various biopsychosocial constructs.nnRESULTS: The BRS is reliable (α = 0.83; Intraclass Correlation Coefficient =0.83; Standard Error of Measurement = 0.33 points; Minimal Detectable Change = 0.91 points) and valid, as all a-priori hypotheses were confirmed. Positive associations were found with measures of self-efficacy and social support, while negative associations were observed with pain intensity, perceived stress, catastrophizing, and symptoms of depression and anxiety. However, the estimated Area Under the Curve (AUC = 0.47) indicated insufficient responsiveness.nnCONCLUSION: While the BRS demonstrated adequate reliability and construct validity in estimating resilience levels in individuals with persistent RCRSP, it lacks sufficient responsiveness in this population.},

abstract = {The evolution from disturbed brain activity to physiological brain rhythms can precede recovery in patients with disorders of consciousness (DoC). Accordingly, intriguing questions arise: What are the pathophysiological factors associated to disrupted brain rhythms in patients with DoC, and are there potential pathways for individual patients with DoC to return to normal brain rhythms? We addressed these questions at the individual subject level using biophysical simulations based on electroencephalography (EEG). The main findings are that unconscious patients exhibit a loss of excitatory corticothalamic synaptic strength. Synaptic plasticity in this excitatory corticothalamic circuitry facilitates the return of physiological brain rhythms, characterized by the reappearance of spectral peaks and flattening of the aperiodic (1/f) component of the power spectrum, in the selection of patients with DoC, particularly in those who are minimally conscious. The extent to which this occurred was correlated with cerebral glucose uptake. The current findings emphasize the importance of excitatory thalamocortical activity in reestablishing normal brain rhythms after brain injury and show that biophysical modelling of the corticothalamic circuitry could help select patients who might be potentially receptive to treatment and undergo plasticity.},

We sought to determine whether clinically measurable asymmetries could predict knee loading symmetry (LSI) during running after ACL-R.nnDESIGN: Longitudinal laboratory study.nnSETTING: Three-dimensional running biomechanics.nnPARTICIPANTS: 24 participants post-ACL-R and 24 healthy, matched controls.nnMAIN OUTCOME MEASURES: LSI of knee loading variables (patellofemoral and tibiofemoral joint reaction force [PFJRF/TFJRF], and knee negative work) were assessed alongside clinically measurable variables (stance time, peak knee flexion angle, knee flexion excursion and foot strike angle). Assessments were conducted at baseline (D1) in both groups, with the ACL-R group also assessed after a 14-day return-to-run program (D14).nnRESULTS: Peak knee flexion angle LSI significantly predicted PFJRF and TFJRF peak and average loading rate LSIs (all p < 0.01). Knee flexion excursion LSI predicted knee negative work LSI (p < 0.001). The R for the predictive equations for PFJRF Average loading rate, PFJRF Peak, TFJRF Peak, TFJRF Average loading rate and Knee negative work were 0.69, 0.75, 0.73, 0.58 and 0.41, respectively.nnCONCLUSIONS: Knee joint loading symmetry during running following ACL-R can be estimated using clinically accessible variables. Among these, peak knee flexion angle symmetry emerged as the most promising predictor.nnLEVEL OF EVIDENCE: Longitudinal observational study, level III.},

abstract = {BACKGROUND CONTEXT: While abundant literature supports the clinical benefits of surgery for degenerative cervical myelopathy (DCM), limited evidence exists regarding the extent to which patients' preoperative expectations are met postoperatively.nnPURPOSE: This study primarily aimed to report the fulfillment of preoperative expectations 12 months postsurgery. Secondary objectives included identifying predictors of expectation fulfillment and overall patient satisfaction following surgical intervention for DCM.nnSTUDY DESIGN/SETTING: We conducted a multicenter analysis of spine surgery patients enrolled in the Canadian Spine Outcomes and Research Network (CSORN)-DCM prospective cohort.nnPATIENT SAMPLE: Patients who underwent surgical treatment for DCM between April 2015 and September 2022 were included.nnOUTCOME MEASURES: Patients completed a preoperative 11-domain expectation questionnaire, quantifying their expected clinical changes in each domain. At 12 months postoperatively, patients reported the extent to which their preoperative expectations were met in each domain, as well as their overall satisfaction with surgical outcomes.nnMETHODS: Multivariable logistic regression models were employed to identify and validate variables associated with expectation fulfillment and satisfaction.nnRESULTS: Among the 490 patients included, 49.2% had all their expectations met and 3.7% had none of their expectations met. The expectations that were most frequently met were prevention of neurological decline (87.5%) and maintaining of independence (86.7%). There were 83% of patients who were satisfied at 12 months. Multivariable logistic regression identified preoperative variables (not smoking, not using pain medication presurgery, having lower levels of depressive symptoms (PHQ-8)), surgical variables (anterior and posterior nonfusion approaches), the absence of peri‑operative adverse events and improvement in pain and Health-Related Quality of life scores at 12 months (NRS neck-pain, SF12 PCS and SF-12 MCS scores) as predictors of expectation fulfillment. Improvement in the mJOA score was not a predictor of expectation fulfillment. Having preoperative expectations met was a strong predictor of being satisfied with the surgery (p<.01).nnCONCLUSIONS: The majority of patients undergoing surgery for degenerative cervical myelopathy (DCM) reported satisfaction with their outcomes, with nearly half experiencing complete fulfillment of their preoperative expectations. Importantly, we identified 9 independent predictors of expectation fulfillment, which in turn emerged as a strong determinant of overall patient satisfaction. These findings highlight the critical role of personalized preoperative counseling in optimizing surgical outcomes. By aligning patients' expectations with realistic goals through individualized care strategies, clinicians may significantly enhance patient satisfaction and the perceived value of surgical intervention in DCM.},

abstract = {Loss of GABAergic and glycinergic inhibitory efficacy in the spinal dorsal horn is associated with neuropathic pain and opioid-induced hyperalgesia in rodent models. Downregulation of the KCC2 chloride extrusion transporter is a key mechanism underlying this decreased inhibitory efficacy, but to-date there is no evidence supporting or opposing this hypothesis in humans. Here, we demonstrate that KCC2 expression is decreased in superficial dorsal horn neurons of organ donors who died with a documented history of pain, or of long-term opioid use. We show profoundly decreased KCC2 dorsal horn membrane expression in a primary cohort associated with either chronic pain or opioid use, and in a replication cohort of mixed chronic pain and opioid use history. These results show that decreased dorsal horn inhibitory efficacy likely promotes chronic pain in humans and support the development of therapeutics augmenting KCC2 function as a treatment for chronic pain and opioid use disorders.},

abstract = {BACKGROUND: Musculoskeletal disorders (MSKDs) are a leading cause of pain and disability, placing a substantial burden on health care systems. Optimizing resource use through innovative interventions is essential. Evidence from randomized controlled trials suggests that not all individuals with MSKDs require ongoing follow-up with a health care provider; for many, education alone is sufficient for symptom resolution. A stepped care model, which prioritizes patient education as a first-line intervention and reserves usual care for those with persistent symptoms, may enhance health care efficiency and reduce costs.nnOBJECTIVE: The primary objective of this randomized controlled trial is to evaluate the effectiveness of a stepped care model compared to the 2 most common approaches for managing MSKDs: usual medical care and usual rehabilitation care. A secondary objective is to assess cost-effectiveness.nnMETHODS: This pragmatic, noninferiority, multiarm, parallel-group randomized controlled trial will enroll 369 adults with MSKDs, randomly assigned to one of three 12-week intervention groups: stepped care, usual medical care (physician-led), or usual rehabilitation care (physiotherapist-led). Participants in the stepped care group will first complete a 6-week education program. Those with persistent symptoms after 6 weeks will receive rehabilitation interventions, while participants whose symptoms have resolved will receive no further intervention. The primary outcome is functional limitations at 24 weeks. Secondary outcomes include pain severity, health-related quality of life, pain-related fear, and pain self-efficacy, assessed at baseline and at 6, 12, and 24 weeks. Linear mixed models will be used for group comparisons, and incremental cost-effectiveness analyses will evaluate cost-effectiveness. The ethics committee of the CIUSSS-CN approved the project (#2024-2982). Findings will be shared through clinical and community platforms, peer-reviewed publications, and conference presentations.nnRESULTS: The Edu-First trial is funded by a project grant from the Canadian Institutes of Health Research (grant #495615). Recruitment began on January 31, 2025. As of September 2025, a total of 65 participants have been enrolled. Recruitment is expected to continue for up to 3 years, targeting approximately 10 new participants per month, and is anticipated to be completed by Winter 2028.nnCONCLUSIONS: We anticipate that the stepped care model will be noninferior to usual medical care and usual rehabilitation care in terms of treatment effectiveness. Furthermore, it is expected to be cost-effective by reducing reliance on expensive resources, such as provider consultations and medical investigations. By emphasizing education and self-management as the initial approach, the stepped care model may enhance access to care without compromising quality, while empowering patients to actively manage their condition. abstract = {Disorders of consciousness (DoC), including unresponsive wakefulness syndrome (UWS) and minimally conscious state (MCS), have limited treatment options and are characterized by low complexity of brain activity. Recent research suggests that psychedelic drugs, which enhance the complexity of brain activity, could offer promising therapies. Here, individualized whole-brain computational models are developed for patients with DoC, optimized with empirical functional magnetic resonance imaging data and diffusion-weighted imaging data, upon which the administration of lysergic acid diethylamide (LSD) and psilocybin is simulated. An in silico perturbation protocol is applied to assess brain dynamics, first distinguishing between different states of consciousness, including DoC, anesthesia, and the psychedelic state. Then, brain dynamics are assessed before and after a simulation of psychedelic drugs on patients with DoC. Findings indicated that the simulation of LSD and psilocybin shifted the brain activity of patients with DoC closer to criticality (the point at a phase transition between order and chaos), with a greater effect in patients in the MCS. In patients with UWS, the treatment response correlated with structural connectivity, while in patients in the MCS, it aligned with baseline functional connectivity. These results offer a computational foundation for using psychedelics in DoC treatment and highlight the potential future role of computational modeling in drug discovery and personalized medicine.},

abstract = {OBJECTIVES: Spectrotemporal acoustical markers enable humans to distinguish between speech and song across diverse cultures. Prior research revealed an asymmetric sensitivity in auditory processing: melody perception is more susceptible to spectral degradation, whereas speech sentence perception is more affected by temporal degradation. However, these findings have primarily been based on non-tonal languages, raising questions about how tonal languages, such as Mandarin, might influence these sensitivity patterns. This study investigates how Mandarin speakers process spectrotemporal features in speech and melody, addressing whether tonal language experience modulates the asymmetric spectrotemporal modulation sensitivity patterns observed in non-tonal languages.nnDESIGN: Twenty-five Mandarin-speaking participants were recruited in the main experiment to discriminate speech or melody content of Mandarin songs under conditions of varying spectral or temporal degradation. An additional 25 participants in the control experiment listened to degraded hummed melodies or spoken sentences under similar degradation conditions.nnRESULTS: Consistent with findings in non-tonal language speakers, Mandarin listeners were more sensitive to temporal degradation in speech and spectral degradation in melody. In addition, they exhibited heightened susceptibility to spectral degradation around 2 cyc/kHz in melody perception, indicating enhanced pitch sensitivity in music, possibly due to tonal language experience. Control analyses revealed reduced sensitivity when hummed melodies or spoken sentences were presented in a single domain, suggesting the influence of cognitive load on auditory processing.nnCONCLUSIONS: These findings underscore the quasi-universality of spectrotemporal cues in speech and melody perception across languages. However, tonal language experience subtly enhances spectral processing in music, providing new insights into the interaction between speech and music perception across different linguistic backgrounds.},

abstract = {Disorders of consciousness (DoC) encompass a range of states characterized by prolonged altered awareness due to heterogeneous brain damage and are associated with highly diverse prognoses. However, the neural mechanisms underlying such diverse recoveries in DoC remain unclear. To address this issue, we analyzed direct recordings from the central thalamus (CeTh), a key hub in arousal regulation, in a series of 23 DoC patients receiving deep brain stimulation treatment (CeTh-DBS). We identified a core set of electrophysiological features of the CeTh, particularly those of the theta rhythm. These features could account for individual recovery outcomes across highly varied etiologies (trauma, brainstem hemorrhage, and anoxia), and across clinical baselines and patient ages. CeTh activities also identified two subgroups of patients with recovery potential, including those with poor initial clinical manifestations but who eventually exhibited functional recovery. A biophysical model further revealed the neurodynamics of the theta rhythm in the CeTh across different brain states correlating with varying consciousness levels. These findings uncover a shared CeTh mechanism underlying diverse recoveries in DoC.},

abstract = {Myotonic Dystrophy Type 1 (DM1) is a complex multisystemic genetic disorder caused by CTG repeat expansions in the gene, leading to RNA toxicity and widespread splicing defects. These splicing abnormalities affect multiple systems, including the respiratory, skeletal, cardiac, nervous, and endocrine systems, resulting in aggressive symptoms that significantly impact quality of life and survival. Cardiac complications are the second leading cause of deaths in DM1, after respiratory insufficiency. Current research is largely focused on understanding cardiac pathology in DM1. This review highlights recent advancements in the clinical and pathological characterization of DM1 cardiac involvement, preclinical models used to study cardiac dysfunction, and emerging therapeutic strategies that target the molecular basis of DM1. Promising approaches include RNA-targeting strategies such as antisense oligonucleotides (ASOs), gene-editing tools like CRISPR-Cas9, and small molecules that modulate RNA splicing. ASOs aim to reduce toxic RNA accumulation, CRISPR-based approaches aim to excise or correct the expanded CTG repeats, and repurposed small-molecule drugs, such as vorinostat, tideglusib, and metformin, could serve as potential therapeutic agents for DM1 patients with cardiac complications. Despite this progress, several challenges remain: the heterogeneity of cardiac manifestations, unpredictable and often silent progression of arrhythmias, limited therapeutic options beyond implantable cardioverter-defibrillator (ICD)/pacemaker implantations, and complex interplay with the multisystemic nature of DM1. More research and well-designed clinical trials are urgently needed to translate these promising strategies into effective treatments for DM1-associated cardiac disease. Here, we discuss the current knowledge in DM1 cardiac pathology and preclinical models as well as the benefits and pitfalls of the available therapeutic approaches.},

abstract = {BACKGROUND: In Canada, more than 4,500 people die by suicide annually, with individuals diagnosed with psychotic disorders being at significantly higher risk. Although the risk factors for suicide in this population are well-established, the assessment of suicide risk remains underexplored.nnAIM: This study examines the practices of mental health professionals working with clients with a psychotic disorder in relation to suicide risk assessment, using the Theory of Planned Behavior (TPB) as a theoretical framework.nnMETHOD: A survey of 148 professionals across Canada was conducted to assess factors influencing the frequency and thoroughness of suicide risk assessments.nnRESULTS: Despite our perceived behavioral control scale having psychometrical flaws, results revealed that social norms were a significant predictor of systematic suicide risk assessments. Professionals identified a lack of time, training, and inadequate clinical tools as major obstacles to thorough assessments. Despite the widespread availability of suicide risk assessment training, many professionals did not feel adequately prepared or confident in conducting such assessments, particularly with clients with a psychotic disorder.nnCONCLUSION: The findings highlight the need for enhanced support, training, and organizational changes to improve the systematic assessment of suicide risk in this vulnerable population.},

abstract = {Although an emerging body of evidence suggests that abnormal forms of tau are present and contribute to Huntington's disease (HD)-a genetic neurodegenerative disorder primarily characterized by the aggregation of the mutant huntingtin (mHtt) protein affecting cognitive, motor, and psychiatric function-it is not clear to what extent this is relevant to the disease phenotype, and hence future treatments. We therefore generated a novel murine model by crossing heterozygous zQ175 knockin HD mice with homozygous tau knockout mice (mTKO). Tau deletion exacerbated both motor and cognitive deficits in zQ175/mTKO mice, which was accompanied by increased mHtt aggregation and alterations in microtubule dynamics, including dysregulated expression of microtubule-associated proteins, aberrant perinuclear β-tubulin accumulation, and microtubule destabilization. Combined, our findings unveil a previously unrecognized protective role of non-hyperphosphorylated tau in maintaining cytoskeletal homeostasis in HD and highlight a functional overlap between tau and huntingtin in regulating aggregate dynamics and cytoskeletal integrity. Our findings complement previous reports suggesting that reducing tau levels could mitigate disease pathology in HD mouse models.},

abstract = {Health and social services nonprofit workers face important occupational challenges that were exacerbated by the COVID-19 pandemic and that may have impacted their psychological health during that time. In Quebec, Canada, many of these workers are employed in community-based organizations providing essential services to vulnerable populations. This longitudinal study examined the temporal evolution of well-being and burnout, as well as their associated predictors, among health and social services community workers across three time points: November 2019 to January 2020 (Time 1), June to September 2020 (Time 2), and January to March 2021 (Time 3). Findings revealed significant decreases in well-being and increases in burnout over time. Reduced opportunities to use one's strengths at work was associated with declining well-being, while increased overcommitment was associated with rising burnout. Contrary to expectations, perceived workload and dissatisfaction with pay decreased during the pandemic. Similarly, higher baseline resources and demands were unexpectedly associated with a greater decline in well-being and increase in burnout. This is one of the few studies to have longitudinally examined the well-being and burnout of health and social services community workers. The results show that, among these workers in Quebec, in a crisis situation such as COVID-19, well-being decreases while exhaustion increases. Strengths used at work and overcommitment seem to be important variables to consider in explaining these results, and such insights can be used for developing targeted interventions to support the psychological health of these essential workers.},

abstract = {IMPORTANCE: Corticobasal syndrome (CBS) is a rare tauopathy, with a complex pathophysiology that usually includes neuroinflammation. Parkinsonism, cognitive impairments, and sleep disturbances are common in CBS, although alterations in sleep architecture remain poorly characterized. Regular exercise has been recommended in CBS to manage gait dysfunction, balance issues, and cognitive decline. However, the effects of regular exercise on sleep quality, sleep architecture, and systemic inflammation in CBS remain unclear.nnOBJECTIVE: The purpose of this study was to describe the effects of regular exercise in CBS.nnDESIGN: The design of this study was a case report.nnSETTINGS: This study was conducted in an academic laboratory.nnPARTICIPANTS: An individual with CBS participated in this case study.nnINTERVENTION: The participant completed a 12-week multimodal training program.nnMAIN OUTCOME(S) AND MEASURE(S): Cardiorespiratory fitness level was assessed with a symptom-limited cardiopulmonary exercise test and strength with a submaximal 1-repetition maximum test. Subjective and objective sleep quality were assessed using the Parkinson Disease (PD) Sleep Scale-2 and actigraphy, respectively. Sleep architecture was evaluated with polysomnography. Cognition and motor function were assessed with the Scale for Outcomes in PD-Cognition and Movement Disorder Society-Unified Parkinson's Disease Rating Scale part III (MDS-UPDRS-III), respectively, functional mobility with the Time Up and Go (TUG), and fatigue with the PD Fatigue Scale. Concentrations of inflammatory markers, including interleukin (IL)-1β, IL-6, IL-10, tumor necrosis factor, and C-reactive protein, were measured from serum collected after a 12-hour fasting period.nnRESULTS: Following the training program (34 sessions; 25.35 hours), improvements in fitness, objective sleep quality and architecture, cognition, TUG, and a reduction in systemic inflammation were observed. Conversely, MDS-UPDRS-III scores deteriorated, and the participant reported diminished subjective sleep quality and increased fatigue.nnCONCLUSIONS: These results, which should be interpreted with caution, suggest that various clinical outcomes improved following multimodal training. Controlled studies are warranted to confirm these observations.nnRELEVANCE: This is the first case report describing the effects of a training program on sleep architecture and systemic inflammation in CBS.},

abstract = {INTRODUCTION: Since 2019, SARPEP (Système Apprenant Rapide pour les programmes de Premiers Épisodes Psychotiques), a rapid learning health system (RLHS) for Quebec's Early Intervention for Psychosis Services, operates to bridge the evidence-practice gap across the province. Despite strong stakeholder support and government recommendations, peer support services remained poorly available. To address this gap, since 2023, the PAIRPEP project was co-developed to support and evaluate the implementation of peer support and family peer support. This paper describes the co-designed study protocol, embedded within this RLHS.nnMETHODS: This participatory, mixed-methods study aims to examine the implementation of the PAIRPEP intervention longitudinally over 3 years across 12 Early Intervention Services and its impact on multiple stakeholders. Informed by the Medical Research Council framework for complex interventions, the project includes a co-designed (with multiple stakeholders) multimodal capacity-building program with specific components developed to overcome barriers to integrating peer support and family peer support. Quantitative questionnaires are collected every 4 months from clinicians while youth and families can complete surveys at any convenient time, via QR codes available in clinics, through the RLHS electronic platform. Focus groups are conducted annually over 3 years with eight stakeholder groups. The analysis integrates findings using thematic synthesis and joint displays to assess convergence and divergence across methods and perspectives.nnRESULTS AND CONCLUSION: This protocol paper outlines the study's co-design, procedures and anticipated contributions. Embedding large-scale innovative intervention implementation (such as peer support) within an RLHS can foster real-time feedback, iterative refinement and inform clinical practice and policies.},

abstract = {An all-encompassing framework seems necessary for understanding brain health in the context of neurodegeneration, particularly due to Alzheimer's disease (AD). We argue that current views, dominated by the amyloid-beta hypothesis, oversimplify the complexity of brain degeneration. We propose a multi-scale, multi-entity approach, treating the brain as a complex system composed of interacting subsystems across various scales, from the nanoscale (genes and proteins) to the macroscale (cognition) and beyond. By redefining brain health through mathematical complexity, we emphasize the importance of integrating diverse biological, environmental, and lifestyle factors to account for the heterogeneity in AD presentations. This framework suggests that failures at different levels of brain function can lead to cascading effects that influence neurodegenerative trajectories. Additionally, it calls for a shift toward personalized treatment approaches that target multiple yet specific pathological mechanisms affecting an individual. We propose computational models as essential tools for simulating and testing these complex interactions. Through this framework, we aim to provide a deeper understanding of neurodegeneration, and advocate for more comprehensive and multi-factorial approaches in both research and clinical practice to advance the treatment of brain health disorders such as AD.},

The ultrastructural properties and plasticity of these synapses and whether they are distinct from those formed by early-born, resident GCs remain unknown. In the present study, we assessed the ultrastructural properties of dendro-dendritic reciprocal synapses formed by either adult-born or resident, early-born GCs with principal neurons in mice of both sexes and studied their plasticity following sensory deprivation and odor learning. The synapses formed by either adult-born or resident, early-born GCs with principal bulbar neurons were undistinguishable in terms of area, diameter, and aspect ratio. In contrast, the synapses formed by adult-born GCs were characterized by a smaller synaptic cleft and a larger density of total and docked synaptic vesicles than synapses formed by resident GCs. Sensory deprivation and odor learning decreased and increased, respectively, the overall density and the number of docked synaptic vesicles in adult-born and resident GCs forming synapses with principal neurons. Our results revealed important differences in the ultrastructural properties of synapses formed by interneurons born at distinct developmental stages and revealed their plasticity in response to sensory deprivation and odor learning. The adult-born neurons in the olfactory bulb play an important role in the bulbar network functioning and odor behavior. In this work, we characterized the ultrastructural properties of synapses formed by adult-born and resident or early-born interneurons with bulbar principal cells and assessed their plasticity following sensory deprivation and odor learning. We revealed marked differences in synapses formed by adult-born interneurons that are characterized by increased density of synaptic vesicles and higher number of docked vesicles as compared to synapses formed by resident, early-born interneurons. We also showed their plasticity following sensory deprivation and odor learning. Our data provide insights for exquisite implication of adult-born interneurons in the odor information processing and odor behavior.},

abstract = {ObjectiveOlder adults with severe mental illness (SMI) represent a complex population with various healthcare needs, even more so when they subsequently develop dementia. While home care (HC) services are advocated for both patients with SMI and dementia, little is known regarding real-life practices, especially for individuals having both conditions. Therefore, we aimed to describe healthcare use and transitions in older adults with SMI across HC user profiles, before and after an incident dementia diagnosis.MethodWe used a retrospective cohort study drawn from Quebec health administrative data on individuals with SMI living in the community, aged 65 and older, and who received a first dementia diagnosis between 2013 and 2015. We described healthcare use 8 months prior and 2 years after the diagnosis, including hospital admissions, visits to the emergency department (ED), and long-term care (LTC) placement.ResultsA total of 3,713 individuals were included, 53% of whom were already receiving HC services before the diagnosis (Group 1), 28% received HC services only after the diagnosis (Group 2), and 19% did not receive any HC (Group 3). While Group 1 showed the highest overall healthcare use before the diagnosis, the most striking increase after the diagnosis was observed for Group 2, catching up with Group 1's levels for many indicators, and even surpassing them in some cases. HC was mainly introduced in the four months following the diagnosis in Group 2. Group 3, while showing the lowest healthcare use throughout the study period, had the second highest mortality rate after Group 1. Groups 2 and 3 were transferred to LTC and died at younger ages than Group 1, in average.ConclusionsThis study highlights potential missed opportunities for intervention, such as an earlier HC introduction which could contribute to prevent an increase in hospitalizations and ED visits, or any HC in Group 3 to mitigate mortality risk and postpone LTC placement.},

abstract = {Perceived social isolation (PSI) predicts adverse health and behaviour problems better than objective social isolation (OSI). PSI is related to higher sensitivity to negative emotional states and emotional dysregulation. Some studies suggest that the more socially isolated people feel, the more sensitive they are to the suffering of others, yet the less empathic they are. However, no study has tested the association between social isolation and empathy while distinguishing their state and trait forms. This study examined how individuals with high- and low-PSI trait perceived and empathized with virtual agents displaying facial expressions. Sixty-six healthy young adults (38 women) were equally divided into high- or low-trait PSI subgroups. All participants reported their current state of PSI and then watched short videos of virtual agents showing facial expressions of physical pain or sadness. For each video, participants rated the perceived intensity of affective states and empathy felt (state empathy). They also completed questionnaires on trait empathy, OSI, moods, and depressive symptomatology. The results showed no effect of group (high- or low-trait PSI), facial expression (pain, sadness), or their interaction on state empathy and intensity mean ratings. However, regression analyses revealed that OSI specifically predicted perspective-taking and empathic concern aspects of trait empathy, whereas trait PSI predicted the personal distress aspect. These findings suggest that social skills may play a role in maintaining and developing relationships but are not linked to one's perception of their quality. This work highlights the importance of differentiating objective from perceived social isolation, given their distinct associations with specific social processes and skills, such as empathy. This distinction could further help develop more targeted interventions to alleviate social isolation.},
